The veins on the eyballs are too big.  If they are going to be larger they should bulge. Also the pattern matches with both eyes, should be varied to look more convincing.

Although the irises on the eyes are very well done, the veins are startlingly
large and draw attention away from nearly everything else. The stuff in the
background is maybe a little TOO blurred.

This is ALMOST an outstanding image.  Everything is technically well-done, and appropriate, but something doesn't quite work here.  Maybe it's the murkiness of the brain inside the cylinder.  Or the bulging veins in the eyes.  Somehow the image falls just short of the mark of serious dark horror.  Instead, it remains in the realm of cartoonish whimsy.  Good work and atmosphere anyhow.

a bit more to the point, I went to read how you created the image and it started by saying what the image was about all over again, so I don't really know how you made this as I stopped reading.
